2024 OHHF Holiday Gift Drive Application for Chicago Families

Overview

We welcome outpatient heart individuals and families to apply to receive gifts for the holiday season. Please note that assistance is only open to our local Chicago market at this time. You or your heart warrior must be outpatient as of October 31st to receive gifts. We will notify you if your application is able to be fulfilled this season or if you are placed on our wait list.

Gifts will be shipped directly to the mailing address provided below and may arrive at separate dates and times due to the nature of the vendor and shipping times.

We ask that you please fill out information for each member of your household and provide a separate wishlist link for each person
